At Innovative Home Health Care, our mission is to provide innovative and effective home health care. We understand the importance of being healthy and value a sense of independence. Our goal is to provide the highest quality medical care to facilitate a smooth transition from hospital to home as well as prevent future hospitalization.
Skilled Nursing
Our team of highly trained and experienced Registered Nurses (RN) and Licensed Vocational Nurses (LVN) aim to restore our patients to their highest level of independence through health teaching and training. We partner with treating physicians to create an individualized Plan of Care.
Speech Therapy
Our certified speech-language pathologists assist patients with speech disorders, swallowing, memory problems, and cognitive difficulties. Our goal is to educate our patients and their loved ones on best practices for continued improvement with speech and language challenges to aid in their recovery through extensive treatment plans.
Physical Therapy
Our certified physical therapists provide rehabilitation therapy for patients in their homes. Patients receive physical therapy to improve strength and balance, gait training, and therapy needed for daily activities. Our goal is to enhance patients’ quality of life and foster safe independent living.
Clinical Social Work
We provide comprehensive in-home medical social work with the goal of improving patients’ emotional, psychological, and social wellbeing by assessing individual needs. Each medical social worker works closely with every patient’s homecare team so they can ensure their plan of care will lead to improved health and independence.
Occupational Therapy
Our certified occupational therapists focus on assisting patients with restoring the function of activities of daily living (ADL). This includes training to increase the patient’s independence with dressing, bathing, combing hair, brushing teeth, and various other activities in their home.
Home Health Aide
Our Certified Home Health Aides (CHHA) provide assistance with bathing and personal hygiene. Our team will coordinate a CHHA Plan to address the needs of the patient. We ensure our patients are treated with the kindness, compassion, and respect that they deserve. Our goal is to increase the independence of each patient.
